Welcome to the High School  Health Office

WINTER 2023/2024 SPORTS ONLINE REGISTRATION IS NOW OPEN

Please have your student bring the forms to the nurse.

Go to our Sports Online Registration (OR) link. Note: You must register online and submit Physical and History forms to the nurse. ** You can not upload physicals, original documents must be turned in to the health office.

Completing Sports Paperwork:

Parents complete the Pre-Participation History Form and Physicians must complete the entire Physical Form on pages 3 and 4 in the Athletics Sports Physical and Health History Update Form.

Health History Form, completed by the parent, and physical form, completed by the physician, are good within 365 days of the start of the first day of practice of the sport.

You must use the State Physical Form linked above, no exceptions. Please make sure the date of the physical exam is clearly labeled (this may be different from the day the Physician signs the form). The Physician must fill out the form completely including blood pressure, pulse, and vision (common areas they tend to leave blank).

If the Physical Form is not completely filled out, your forms will be returned to you causing delays. We can not process incomplete forms. NO EXCEPTIONS.

If possible, please try to get your sports physical evaluation completed by your Family Physician. The Moorestown Preparticipation Physical Packet is mandatory for participation in interscholastic sports if your current physical was administered more than 365 days before the start of your Fall Sport’s first practice.
