MHS Summer School Brochure:
Students who have failed a course with a final grade of "E" (50% to 59%) and need to restore their credits may be able to take remediation courses through summer school.  Download the 
Summer School Brochure 2019 for more information.  Since failed courses could affect your graduation requirements, NCAA eligibility, and NJSIAA eligibility (high school sports) contact your Guidance Counselor with any questions.

The PLO Application Form 2014 is for use when requesting approval for taking courses outside MTPS, internships or personal learning opportunities as outlined in the PLO Document and the MHS Program of Studies.

College Forms:

Parent Comments and Observation Sheet
Helps parents of Seniors participate in their child's post-secondary planning process by providing your child's counselor with supporting information that may be included in the counselor's letter of recommendation.

Student Activities Sheet
Allows Seniors to provide their counselor  with a list of their school related and non-school related activities and accomplishments. This information may be included in the counselor's letter of recommendation.

Transcript Release Form
To release a transcript, a Transcript Release Form must be signed by a parent/guardian or an adult student (age 18) and given to the School Counseling Department. This form can also be used to obtain a copy of your unofficial transcript.

Weekly Progress Report Form
This form is helpful for parents to get periodic feedback from teachers regarding their child's progress. It can be used weekly, by-weekly or as often as needed. It can be used for one class. two classes, or as many as necessary. The student should bring the form to their teacher, then return the form to their parents/

Community Service Request to Record Hours Form
Anyone who has taken the time to help others knows the good feeling one gets from volunteering is reward enough. The willingness to commit time and effort to assist others is a character trait valued by colleges and employers. To recognize student efforts, Moorestown High School students can earn designation on their transcripts for service to the community. Transcript recognition begins at 50 hours per service year and increases in the following increments: 

  •  50+ hours - Outstanding level of service
  • 100+ hours - Exceptional  level of service; President's Volunteer Service Award (Bronze)
  • 175+ hours - Exceptional  level of service; President's Volunteer Service Award (Silver)
  • 250+ hours - Exceptional  level of service; President's Volunteer Service Award (Gold)

In addition to transcript designation, students who successfully record between 50 and 99 hours of community service in one service year will be awarded a certificate denoting an Outstanding level of service for the year. Students who submit 100 or more hours in one service year will be awarded a certificate denoting an Exceptional level of service for the year. In addition to internal recognition, the President's Volunteer Service Award  is presented to students that record 100 or more community service hours in a service year. Each year, the MHS Community Service Council will honor the students who have cumulatively recorded service hours within the top 10% of their graduating class. Membership in an MHS-sponsored club or activity is not a prerequisite for community service recognition.

The 2019-2020 Request to Record Volunteer Hours Form must be used to document service hours from May 1, 2019 through April 30, 2020. Students must submit the Request to Record Volunteer Form to the appropriate grade-level advisor by the specified deadlines. The form must be signed by the supervisor overseeing the service work or project; a signed letter (on the letterhead of the sponsoring organization or agency) attached to the form is acceptable. This Request to Record Volunteer Hours Form can also be obtained from the MHS Website, MHS Guidance Office, MHS Main Office, or outside of the classrooms/offices  of the MHS Community Service Council advisors. It is recommended that students submit their hours as earned and contact their grade-level advisor to confirm their recorded service hours on a periodic basis.

The Moorestown High School Community Service Council allows students to record volunteer hours for unpaid services that:

  1. Directly benefits people that are experiencing poverty, discrimination, abuse, poor health, old age, and/or physical-mental limitations;
  2. Provides academic support or other enrichment activities for students or community members;
  3. Relates to public safety or the protection of animals/the environment;
  4. Enhances the betterment of the community through community-building activities; or
  5. Supports the mission of a non-profit organization.

Please Note: Community service hours will not be awarded for in-kind donations or travel to-and-from a service activity.

Graduating Seniors:

Attention:  MHS Seniors and Parents - 2018 Senior Sign-Off Form:
Graduation is around the corner and it is time to fill out your MHS Graduation Sign Off Form.  This year, the form is in Genesis. 

Students, if you will be age 18 by May 31, the form has been pushed out to your student portal account.  If not, it has been pushed to your parents’ account and one of your parents will need to fill out and submit the form in Genesis.  The link for both students and parents to access Genesis is:  https://parents.mtps.com.  If you do not remember your Genesis password, please click the ‘Forgot password’ link and you will receive an e-mail to reset your password.

If you cannot complete the form electronically, please stop down to the Guidance Office to get a paper form, but we are requesting that you complete this electronically if possible.

Genesis will be locked until this form is submitted.

*Be sure to give a stamped business-sized envelope, addressed to the college you will be attending, to Ms. Berger or Ms. Waldin.  Check with the college to make sure you are sending the transcript to the correct address.
